陈旦平诊治不孕症经验探析

摘要
[目的]总结与分析陈旦平教授诊治不孕症的临床经验。[方法]跟随陈旦平教授临证,感悟其诊治不孕症的方法,总结其对不孕症病因病机的认识和特色的治则治法,并分析验案一例作为佐证。[结果]陈旦平教授认为不孕症的主要病机在于肾气不充,冲任不调。治疗上以"调经种子"为原则,治"肾"为要,肾、肝、脾三脏同调;强调分期而治,注重运用补肾调周法,创制了特色方药育肾方和温肾方。所举验案中患者诊断为原发性不孕症、多囊卵巢综合征,辨证为肾虚痰瘀证,治以补肾化痰、活血通络之法,予育肾方、温肾方加减,周期调治,患者终有排卵并受孕。[结论]陈旦平教授在治疗不孕症时运用补肾调周,并兼顾肝脾的方法帮助患者排卵种子,获得良效,值得临床推广应用。
[Objective] To summarize and analyze the clinical experience of Professor CHEN Danping in the diagnosis and treatment of infertility. [Methods]Following Professor CHEN Danping's clinical experience, realizing the method of diagnosis and treatment of infertility, Professor CHEN Danping's understanding of the etiology and pathogenesis of infertility and the characteristic treatment methods were summarized, and one case was provided and analyzed. [Results] Professor CHEN Danping believes that the main pathogenesis of infertility is no enough kidney Qi and no adjustment in Chong and Ren. In the treatment, the principle is "regulating menstruation and planting seeds", the treatment of "kidney" is the key, and the kidney, liver and spleen are in harmony; the treatment by stages, and the method of tonifying the kidney and regulating the cycle are emphasized, and the prescription of nourishing the kidney and warming the kidney is created. The case cited was diagnosed as primary infertility and polycystic ovary syndrome, and the syndrome differentiation was kidney deficiency and phlegm stasis. Yushen recipe and Wenshen recipe were periodic used according to therapeutic principles of tonifying kidney, resolving phlegm and activating blood circulation and dredging collaterals. The patients eventually ovulated and conceived,which reflected Professor CHEN Danping's diagnosis and treatment ideas. [Conclusion] Professor CHEN Danping used the method of tonifying kidney and regulating week, and considering the liver and spleen meanwhile to help patients ovulate seeds, achieved good results, which was worthy of clinical application.
