多层螺旋CT扫描在诊断肾脓肿合并肾周感染中的价值分析

摘要
目的探讨多层螺旋CT扫描在肾脓肿合并肾周感染诊断中的应用价值。方法我院收治的37例肾脓肿合并肾周感染患者,均于术前行CT平扫和增强扫描,观察其影像学特征。结果 37例患者CT扫描图像经后处理技术处理,均无阶梯状伪影产生且对肾脏解剖结构、形状、病变情况显示清晰。平扫显示病灶部位呈现出密度均匀的低密度影或密度不均的等密度低密度混合影,病灶多呈较为规则的类圆形,部分于病灶中可见气液平面。增强后病灶边缘低密度区可见环状强化,病灶中心均未明显强化显示。CT平扫与增强扫描对脓肿直径>2 cm、<1 cm的检出情况比较差异无统计学意义(P> 0. 05),对肾脓肿合并肾周感染的检出情况比较,差异有统计学意义(P <0. 05)。结论多层螺旋CT扫描对肾脓肿并肾周感染的检出率高,可清晰显示肾脓肿的图像特征,对提高脓肿并肾周感染的诊断率具有重要价值。
Objective To explore the application value of multi-slice spiral CT scan in the diagnosis of renal abscess and perirenal infection.Methods A retrospective analysis was performed on 37 patients with renal abscess and perirenal infection in our hospital.All patients underwent the CT non-enhanced scan and enhanced scan before surgery.The characteristics of images were observed.Results After process of the CT scan images of the 37 patients by post-processing technology,the images showed no stage-wise artifact and clear indicated the renal anatomy,shape and pathological changes.Non-enhanced scanning showed that the lesion presented uniform low-density shadow or equal density or low density mixed group photo.The lesions were a more regular round shape,and some of them could be seen in the gas-liquid plane.Circular enhancement was observed in the low-density area at the edge of the lesion after enhancement.The differences between CT non-enhanced scan and enhanced scan had no statistical significance in the detection of abscess diameter > 2 cm and < 1 cm( P > 0. 05).However,the differences between CT non-enhanced scan and enhanced scan in the detection of renal abscess with perirenal infection were statistically significant( P < 0. 05).Conclusion MSCT has a high detection rate of renal abscess with perirenal infection,which can clearly show the image features of renal abscess.Therefore,it is of great value in improving the diagnosis rate of abscess with perirenal infection.
