锁定钢板内固定并打压植骨治疗老年肱骨近端骨折的临床效果及优点分析

摘要
目的探究锁定钢板内固定并打压植骨治疗老年肱骨近端骨折的临床效果及优点。方法回顾性分析2015年3月至2016年3月于本院接受锁定钢板内固定并打压植骨治疗的32例老年肱骨近端骨折患者(研究组)和同期行传统切开复位钢板内固定治疗的32例患者(对照组)临床资料。记录两组患者手术时间、术中出血量及术后12个月内并发症发生情况;比较其手术前、手术后12个月时肩关节功能(Neer肩关节功能评分)及肩关节活动范围[关节活动度(ROM)]评估结果差异。结果研究组患者手术时间、手术出血量均明显少于对照组(P<0.05);手术后12个月,研究组临床疗效、肩关节活动(前屈、外展、后伸、内旋、外旋)范围均明显大于对照组(P<0.05),研究组并发症情况明显少于对照组(P<0.05)。结论锁定钢板内固定并打压植骨治疗老年肱骨近端骨折使患者临床疗效更好,预后更好。
Objective To explore the clinical effects and advantages of locking plate internal fixation and impaction bone grafting in the treatment of elderly proximal humeral fractures. Methods Clinical data of 32 elderly patients with proximal humeral fractures who underwent locking plate internal fixation and impaction bone grafting(study group) in the hospital from March 2015 to March 2016 and 32 patients who were given conventional open reduction and internal fixation(control group) were retrospectively analyzed. The operative time, intraoperative blood loss and complications within 12 months after operation were recorded in the two groups. The shoulder function(Neer shoulder function score) and shoulder range of motion [range of motion(ROM)] were compared before operation and at 12 months after operation. Results The operative time and surgical blood loss in study group were significantly less than those in control group(P<0.05). The clinical efficacy and shoulder ranges of motion(flexion, abduction, extension, internal rotation, external rotation) were significantly greater than those in control group(P<0.05). The complications in study group were significantly less than those in control group. Conclusion Locking plate internal fixation and impaction bone grafting for elderly proximal humeral fractures has good clinical efficacy and good prognosis.
