行为改变护理干预可促进股骨颈骨折老年患者的术后康复

摘要
为探讨基于行为改变理论为基础的护理干预在老年股骨颈骨折闭合复位内固定术术后患者中的应用效果及对患者功能锻炼的影响,本研究选取2017年2月至2017年10月在我院接受股骨颈骨折闭合复位内固定术的老年患者为观察对象,并根据随机数字表法将其分为对照组和观察组,两组患者均给予常规护理,观察组在此基础上给予基于行为改变理论为基础的护理干预。观察两组患者干预前后自我效能、功能锻炼依从性、关节功能恢复和生活质量的差异。结果表明:两组患者干预前自我效能和自护能力无差别,干预后,观察组患者自我效能和自护能力高于对照组;观察组患者功能锻炼依从性为97.50%,明显高于对照组(χ~2=5.000, p=0.025);观察组患者术后1个月和3个月的关节功能评分(Harris评分)高于对照组;两组患者干预前生活质量无差别,干预后,观察组患者的生活质量高于对照组(p<0.05)。本研究初步结论证实,以行为改变理论为基础的护理干预可促进老年股骨颈骨折术后患者的术后康复,提高生活质量。
In order to explore the effect of nursing intervention based on behavior change theory in elderly patients with femoral neck fracture after closed reduction and internal fixation, and its effect on functional exercise,The elderly patients who received closed reduction and internal fixation of femoral neck fracture in our hospital from February 2017 to October 2017 were selected as the observation subjects in this research. They were randomly divided into control group and observation group according to the random number table method. The two groups were given routine nursing, and the observation group was given the nursing intervention based on the behavioral change theory. The differences of self-efficacy, functional exercise compliance, joint function recovery and quality of life were observed before and after the intervention of the two groups. The results presented that the two groups of patients before the intervention of self-efficacy and self-care ability difference, intervention, observation group patients self-efficacy and self-care ability is higher than that of control group; The observation group of patients with functional exercise adherence was 97.50%, significantly higher than the control group(χ~2=5.000, p=0.025); The Harris scores of the patients in the observation group were higher than those in the control group at 1 months and 3 months after operation. The quality of life in the two groups was no difference before intervention,and the quality of life of the patients in the observation group was higher than that of the control group(p<0.05). The preliminary conclusions of this study confirm that the nursing intervention based on the theory of behavior change can promote the postoperative rehabilitation and improve the quality of life of the patients.
