琥珀酰明胶注射液治疗低血容量休克所致急性心力衰竭疗效探讨

摘要
目的探究琥珀酰明胶注射液治疗低血容量休克所致急性心力衰竭临床效果。方法选取某医院2015年12月—2017年12月间收治50例低血容量休克所致心力衰竭患者进行实验研究,采用随机数字法将患者分为观察组和对照组,每组25例,对照组采用常规治疗,对照组在常规治疗的基础上采用长源雪安琥珀明胶注射液治疗,比较两组患者治疗有效率、并发症发生率及各项生理指标。结果观察组治疗有效率88.0%明显高于对照组72%,差异有统计学意义(χ~2=12.361,P<0.05),观察组并发症发生率20.0%明显优于对照组4.0%,差异有统计学意义(P<0.05),治疗观察组患者收缩压(58.23±13.24)mmHg、血氧饱和度(0.91±0.15)、心率(121.36±18.45)次/min、尿量(55.21±38.16)mL/h、B型钠尿肽(1452.21±318.31)pg/mL、左室射血分数(0.41±0.07)与对照组患者收缩压(58.41±10.36)mmHg、血氧饱和度(0.82±0.12)、心率(119.41±18.13)次/分、尿量(60.12±35.64)mL/h、B型钠尿肽(1 582.12±387.27)pg/mL、左室射血分数(0.49±0.12)差异无统计学意义(P>0.05),治疗后观察组患者收缩压(95.47±15.24)mmHg、血氧饱和度(0.94±0.07)、心率(96.27±13.64)次/min、尿量(146.21±50.18)mL/h、B型钠尿肽(1 108.27±218.21)pg/ml、左室射血分数(0.60±0.07)明显优于对照组收缩压(86.23±13.24)mmHg、血氧饱和度(0.93±0.07)、心率(85.21±12.36)次/min、尿量(132.45±81.24)mL/h、B型钠尿肽(1 204.12±236.13)pg/mL、左室射血分数(0.56±0.04),差异有统计学意义(P<0.05)。结论低血容量休克所致急性心力衰竭患者采用琥珀酰明注射液进行治疗,可有效缓解患者临床症状,纠正心衰,具有一定临床价值,值得推广应用。
Objective To explore the clinical effect of succinyl gelatin injection on acute heart failure caused by hypovolemic shock. Methods A total of 50 patients with heart failure caused by hypovolemic shock in a hospital from December 2015 to December 2017 were selected for experimental study. The patients were divided into observation group and control group by random number method, 25 cases in each group. The control group was treated with routine treatment, while the experimental group was treated with Changyuan Xue'an Amber Gelatin Injection on the basis of routine treatment. Incidence of complications and various physiological indicators of two groups was compared. Results The effective rate of the observation group was 88.0%, which was higher than that of the control group(72.0%), and the difference was statistically significant(χ~2=12.361, P<0.05). The incidence of complications in the observation group was20.0%, which was higher than that in the control group(4.0%). The difference was statistically significant(P<0.05). The systolic blood pressure(58.23±13.24) mmHg, oxygen saturation(0.91±0.15), heart rate(121.36±18.45) times/min, urine volume(55.21±38.16) mL/h, B-type natriuretic peptide(1452.21±318.31)pg/mL, left ventricular ejection fraction(0.41±0.07 in the treatment group were not significantly higher than those in the control group,(58.41 ±10.36)mmHg(0.82±0.12),(119.41±18.13)times/min,(60.12±35.64)mL/h,(1 582.12±387.27)pg/mL,(10.49±0.127)(P>0.05). After treatment,systolic blood pressure(95.47±15.24) mmHg, blood oxygen saturation(0.94±0.07), heart rate(96.27±13.64)beats/minutes, heart rate(96.27 ±13.64) beats/minutes, urine volume(146.21 ±50.18)mL/h, B natriureturetic peptide(1 108.27 ±218.21)pg/mL, left ventricular ejection fraction(0.60±0.07) in the observation group were significantly better than those in the control group,(86.23 ±13.24)mmHg,(0.94 ±0.07),(85.21 ±12)mL/h,(132.45 ±81.24)mL/L,(1 204.12 ±236.13)pg/m L、(0.56±0.04), the difference was statistically significant(P<0.05). Conclusion Succinimide injection can effectively relieve clinical symptoms and correct heart failure in patients with acute heart failure caused by hypovolemic shock. It has certain clinical value and is worthy of popularization and application.
