腹腔镜下急性胃穿孔手术的临床疗效分析

摘要
目的探讨腹腔镜下急性胃穿孔手术的临床疗效。方法选择90例急性胃穿孔患者据术式分组,对照组开展传统手术治疗,观察组行腹腔镜下急性胃穿孔手术治疗。比较2组急性胃穿孔修补效果;手术中出血总量、胃肠功能恢复时间、总操作时间、术后住院时间;手术前后患者生活质量评分、应激指标;并发症情况。结果观察组急性胃穿孔修补效果高于对照组(P<0.05);观察组手术中出血总量、胃肠功能恢复时间、总操作时间、术后住院时间少于对照组(P<0.05);手术前2组生活质量评分、应激指标相近(P>0.05),手术后观察组生活质量评分、应激指标优于对照组(P<0.05);观察组并发症发生率低于对照组(P<0.05)。结论急性胃穿孔患者行腹腔镜下手术疗效肯定,可有效修补病变,降低并发症发生率,促进患者生活质量改善,并加快康复速度,减轻手术应激,缩短住院时间。
Objective Toinvestigate the clinical effectiveness oflaparoscopic surgeryfor acute gastric perforation.Methods 90 patients with acute gastric perforation from February 2016 to January 2018 were selected according to surgical procedure.The control group underwent traditional surgical treatment.The observation group underwent laparoscopic surgery for acute gastric perforation.The effect of acute gastric perforation repair was compared between the two groups;total hemorrhage during operation,recovery time of gastrointestinal function,total operation time,postoperative hospital stay;quality of life score and stress indicators before and after surgery;complications.Results The effect of acute gastric perforation in the observation group was higher than that in the control group(P<0.05).The total amount of blood loss,recovery time of gastrointestinal function,total operation time and postoperative hospital stay in the observation group were better than those in the control group,P<0.05;The scores of quality of life and stress in the first two groups were similar,P>0.05;postoperative scores of quality of life and stress in the observation group were better than those in the control group,P<0.05.The observation group had fewer complications than the control group,P<0.05.Conclusion Acute gastric perforation in patients with acute gastric perforation surgery is effective,can effectively repair lesions,reduce the incidence of complications,improve patient quality of life,and accelerate the speed of recovery of patients,reduce surgical stress,shorten hospital stay.
