摘要
目的探讨慢性心功能衰竭(CHF)患者血清糖类抗原125(CA125)、B型钠尿肽(BNP)、同型半胱氨酸(Hcy)、成纤维细胞生长因子23(FGF23)水平变化与患者心功能指标的关系。方法选取我院确诊的CHF患者126例(CHF组),选取正常健康人群60例作为对照组,检测研究对象的血清CA125、BNP、Hcy、FGF23水平,采用超声心动图测量两组的左室射血分数(LVEF%)、左室舒张末期内径(LVEDd)、左心室质量指数(LVMI),并探讨血清指标与心功能指标的相关性。结果 CHF组的血清CA125、BNP、Hcy、FGF23水平显著的高于对照组(P <0. 05);Ⅰ级、Ⅱ级CHF组的血清CA125、BNP、Hcy、FGF23水平显著的低于Ⅲ级和ⅣCHF组患者(P<0.05);CHF组患者血清CA125、BNP、Hcy与LVEF%呈负相关关系(P<0.05),血清CA125、BNP、Hcy与LVEDd呈正相关关系(P<0.05),血清BNP、Hcy、FGF23与LVMI呈正相关关系(P<0.05)。结论 CHF组的血清CA125、BNP、Hcy、FGF23水平较正常人群显著的升高并且与超声心动图指标具有一定相关性。
        Objective To investigate the changes of serum carbohydrate antigen 125(CA125),BNP(BNP),homocysteine(Hcy) and fibroblast growth factor 23(FGF23) in patients with chronic heart failure(CHF)and their relationships with cardiac function. Methods 126 cases of diagnosed CHF patients were enrolled in this study as the CHF group,while 60 healthy person were selected as the control group. The serum CA125,BNP,Hcy and FGF23 levels were measured among all patients. For both groups, left ventricular ejection fraction( LVEF%), left ventricular end diastolic diameter( LVEDd) and left ventricular mass index(LVMI) were measured by echocardiography. The correlations between serum indexes and cardiac function was evaluated. Results The levels of serum CA125, BNP, Hcy, and FGF23 in the CHF group were significantly higher than those in the control group( P <0. 05); the levels of serum CA125,BNP,Hcy,and FGF23 in the level I and II of the CHF groups were significantly lower than those in the level III and IV of the CHF groups. Among CHF patients,serum CA125,BNP,and Hcy were negatively correlated with LVEF%(P <0. 05), and positively correlated with LVEDd( P < 0. 05). Also, serum BNP and Hcy were positively correlated with LVMI(P < 0. 05) and serum FGF23 was positively correlated with LMVI( P < 0. 05).Conclusion The levels of serum CA125,BNP,Hcy and FGF23 in the CHF group are significantly higher than those in the control group,and are correlated with the echocardiographic parameters.
