陈明对《伤寒论》五苓散证的解读及运用

摘要
五苓散在《伤寒论》中凡八见,为苓桂剂的代表方剂。本证核心病机为膀胱气化不利,水气内停,核心症状为小便不利、口渴;方药配伍特点:茯苓、猪苓、泽泻利水于下,桂枝化水于外,白术防水于中。陈明教授认为,五苓散证水蓄于内,往往影响全身,上、中、下三焦证候皆可见。水蓄于下,则见小便不利、少腹满;水亏于上,则见消渴,"渴欲饮水,水入则吐";水扰于中,脾胃升降紊乱,则可见心下痞、霍乱等病证。以五苓散治风水、消渴、干燥症、水痞、汗出异常、脑积水等均应收效。
The Prescription of Wu Ling Powder,being the typical one of Ling Gui Prescription,was mentioned 8 times in Shang Han Lun. The core pathogenesis of this syndrome is the dysfunction in bladder gasification,hence Water and Qi are blocked within the body. The core syndromes are unfavorable urinary and thirsty. The compatibility characteristics of the prescriptions are as follows: Poria cocos,Porcupine,and Alisma orientalis can dispel the Water to go down;,Cassia Twig can sperse the Water outside of the body; Atractylodes prevents the Water from stopping within the body. Professor Chen Ming believes that the Water within patients with Wushen San Syndrome often affects the whole body. The symptoms in upper,middle and lower burner are visible. When the Water is stored in the lower part,it will be unfavorable for urination and there will be full of belly; if the water is not enough in the upper side of body,then thirst would happen. " When thirst patients like to drink water,yet water intake would be vomited";Water disturbs in the middle part of the body,the Spleen and Stomach Qi would in disorder,people suffer from stuffy below heart and Cholera. Therefore,the treatment of Wind Water Syndrome,thirst,dryness,Water lump,abnormal sweating,hydrocephalus,etc. with Wu Ling Powder should be significantly effective.
